Your Message:

Thirteen characters per line. Maximum of three lines.
Characters include: letters, numbers, spaces and punctuation marks.

We will not be able to accommodate any specific placement requests.

Bricks for placement at Veterans Park are $100 each.
Please submit a separate form for each brick you would like to purchase.

A personal keepsake brick may also be purchased for $50 each.

After your brick request is submitted, a Skokie Park District representative will contact you confirm your order and arrange payment. Bricks will be commissioned upon receipt of payment in full.

A gift letter will be sent to you when the brick has been completed with information regarding your brick placement. If you ordered an additional keepsake brick, you will be notified when and where to pick it up.

Proceeds to benefit Youth Foundation of Skokie, a 501(c) 3 charitable organization. All donations are deductible to the fullest extent of the law. A portion of your donation will go to the Illinois AMVETS’ Dreams Unlimited Program for terminally and seriously ill children of Veterans and Service Members.
